Use this sheet when contacting a community nursing provider. Keep it for your own comparison; you do not need to share it with DisabilityChoice. Ask the provider to explain anything that is unclear.
Provider and conversation
Provider / branch:
Person spoken to / date:
1. Services and availability
- Can you provide the type of nursing support I need?
- Is this an assessment, ongoing care, training or another service?
- Do you visit my postcode, and what travel limits apply?
- When could a suitable nurse actually start?
- Can you offer my required visit days, times and frequency?
Notes:
2. People, continuity and communication
- Who would provide and oversee my support? How do I check their registration?
- What relevant experience do they have?
- Can I usually see the same nurse? Who covers leave or illness?
- Who do I contact if a visit changes or something is not working?
- How will you communicate in the way that works for me?
Notes:
3. A written quote I can compare
- What is the rate for my actual visit times and nursing support?
- Is there a minimum booking, travel charge or other visit cost?
- Are assessment, report writing and follow-up included?
- What are the cancellation and ending-service terms?
- Can you work with my funding arrangement, and what needs checking first?
Notes:
4. Before I agree
- I understand what is and is not included.
- The provider has confirmed suitability, availability and the start arrangements.
- I have checked the funding pathway and what I may need to pay.
- I have an agreement in a format I can understand.
- If changing providers, I have discussed a safe handover with my care team.
