Inquiry form
When you submit an inquiry through our plan manager comparison form, we collect the following information. Fields marked optional are not required to submit the form. By ticking the consent checkbox on the form, you provide express consent to DisabilityChoice collecting and using your information to assess your needs and match you with a suitable plan manager, and to disclosing your information to the matched plan manager so they can contact you about plan management services, as described in this policy. If you are submitting this form for or on behalf of another person, you confirm you have their authority or consent to provide their information and request contact on their behalf.
Contact details
- First name — used to address communications to you
- Email address — used to send your matched provider recommendation and confirmation
- Phone number — required; used by matched providers to contact you directly
- State or territory — used to identify plan managers with coverage in your area
- Suburb or postcode — used to refine location-based matching
- Provider location preference (optional) — whether you prefer a local or national provider
About the participant
- Your relationship to the participant — whether you are the participant, a parent or guardian, family member, support coordinator, or plan nominee
- Participant age group — used to identify providers experienced with the relevant life stage
- Primary disability type (optional) — used to match you with providers who specialise in your condition. You are not required to disclose a diagnosis and may select "Prefer not to say"
Service needs
- Services required — whether you need plan management, support coordination, or both
- NDIS plan status — your current situation (active plan, plan under review, recently approved, awaiting decision, or researching)
Plan details (all optional)
- Approximate NDIS funding amount — used to match you with providers suited to your plan size
- Current plan management type — how your plan is currently managed
- Plan expiry or review timeframe — when your plan is due for review
- Current situation — whether you are seeking your first plan manager, switching, or adding plan management
Matching preferences
- Provider priorities — what matters most to you in a plan manager (e.g. fast invoice payments, multilingual support, local provider)
- Urgency — how soon you need to get started
Contact preferences (all optional)
- Best time to contact you
- Preferred contact method — phone, email, SMS, or video call
- Language preference — if you prefer to communicate in a language other than English
- Additional notes — any other information you choose to provide about your situation or requirements
Technical data
- Source page URL — automatically recorded to identify which page of our site your inquiry came from. This is not linked to your identity and is used for internal reporting only.
A note on sensitive information: Some information you provide — including disability type, age group, and any health-related details in your additional notes — may constitute sensitive information under the Privacy Act 1988 (Cth). By submitting the inquiry form and ticking the consent checkbox, you consent to us collecting and using this information to assess your needs and match you with a suitable plan manager or support coordinator, and to disclosing it to the matched provider so they can contact you about plan management services.
Analytics — Google Analytics 4
We use Google Analytics 4 (GA4) to understand how visitors use this website. GA4 collects anonymised data including pages visited, time spent on pages, approximate geographic location (city level), device type, and browser. This data is processed by Google on servers outside Australia. GA4 does not collect your name or email address. You can opt out of GA4 tracking at any time using the Google Analytics Opt-Out Browser Add-on.
Advertising — Facebook Pixel
We use the Facebook Pixel to measure the effectiveness of our advertising on Facebook and Instagram. The Pixel collects anonymised event data (such as page views and form submissions) and may be used by Meta to show our advertisements to people with similar interests. You can manage your Facebook advertising preferences via your Facebook Ad Preferences settings.
Advertising — LinkedIn Insight Tag
We use the LinkedIn Insight Tag to measure conversions from LinkedIn advertising and to enable demographic reporting about our website visitors. The Insight Tag may collect data including job title, seniority, and industry from LinkedIn members who visit our site while logged in to LinkedIn. You can opt out of LinkedIn's retargeting via LinkedIn's guest controls.
Cookies

How We Use Your Information

- Provider matching — we share your contact details, location, service needs, disability type (if provided), and matching preferences with one or more NDIS plan managers or support coordinators from our comparison panel. This sharing is the core purpose of your inquiry and you consent to it by submitting the form.
- Lead routing and prioritisation — we use your NDIS plan status, urgency, and other signals to determine how quickly your inquiry is routed to matched providers. Inquiries submitted by support coordinators on behalf of a participant are treated as priority referrals.
- Follow-up communication — we may contact you by email to confirm your inquiry was received and to ask whether the matching process was helpful.
- Service improvement — anonymised, aggregated inquiry data (never individual records) may be used to improve our comparison methodology.
We do not sell your personal information to any third party. We do not share your information with providers outside our comparison panel. We do not use your information for direct marketing beyond the scope of your inquiry.
Your Rights Under the Australian Privacy Act 1988
Under the Privacy Act 1988 (Cth) and the Australian Privacy Principles, you have the following rights:
- Access — you may request access to the personal information we hold about you at any time.
- Correction — if you believe information we hold about you is inaccurate, out of date, incomplete, or misleading, you may request that we correct it.
- Complaint — if you believe we have breached the Australian Privacy Principles, you may lodge a complaint with us in the first instance. If you are not satisfied with our response, you may escalate your complaint to the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.

How We Protect Your Information
We take reasonable steps to protect your personal information from misuse, interference, loss, and unauthorised access. Specifically:
- Encryption in transit — all data transmitted between your browser and this website is encrypted via SSL/TLS (HTTPS).
- Server security — this website is hosted on Hostinger servers with access controls restricted to authorised personnel.
- Retention and deletion — inquiry form data is retained for 12 months for the purposes of record-keeping and referral fee reconciliation, then permanently deleted. We do not retain personal information beyond this period unless required by law.
- Third-party processors — providers to whom we pass inquiry data are bound by their own privacy obligations under Australian law and their service agreements with participants.

You can limit the collection of advertising and analytics data through the following methods:
